Logging In
Navigate your browser to forms.altastar.com Enter your User ID and password and click the Login button.
Log in with your User ID, which is not necessarily the same as your email address.
Persons who have been added as an authorized User by the account holder, may log in to Alta Star Forms Online. When a User is added, a User ID is assigned. The account holder provides the User ID to authorized users.
If you don't know your User ID or password, at the login screen click on the "Lost/Change Password/User ID" link. After following the prompts and instructions, you will be able to view/change your User ID.
If your email address is not recognized using the Lost Password link, contact the account holder to verify or update your log in information.
